Radavanta LLC ("Radavanta," "we," "our," or "us") is committed to protecting the privacy and security of your protected health information ("PHI"). In our role as a business associate providing mobile imaging sourcing, staffing, and lifecycle management services to covered healthcare entities, we are required by the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"), the Health Information Technology for Economic and Clinical Health Act ("HITECH"), and their implementing regulations to maintain the privacy of PHI, provide you with this notice of our legal duties and privacy practices, and abide by the terms of the notice currently in effect.
We may use and disclose PHI only as permitted by our Business Associate Agreements with covered entities and as allowed by law, including for the following purposes:
We will not use or disclose your PHI for marketing, sale of PHI, or most uses of psychotherapy notes without your written authorization. You may revoke an authorization at any time in writing, except to the extent we have already relied on it.
Because Radavanta typically acts as a business associate, requests to exercise these rights should generally be directed to your healthcare provider. We will support and cooperate with the provider in fulfilling your request.
We maintain administrative, physical, and technical safeguards designed to protect the confidentiality, integrity, and availability of PHI, including access controls, encryption of data in transit and at rest where applicable, workforce training, audit logging, incident response procedures, and vendor risk management aligned with the HIPAA Security Rule.
We reserve the right to change this notice and to make the revised notice effective for PHI we already have as well as any information we receive in the future. The most current version will always be posted on this page with an updated effective date.
If you believe your privacy rights have been violated, you may file a complaint with Radavanta using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights, at 200 Independence Avenue, S.W., Washington, D.C. 20201, or by calling 1-877-696-6775. We will not retaliate against you for filing a complaint.
